Once your appointment has been schedule with an office representative, you will be emailed a link to set up your REQUIRED patient portal. To set up the portal, Click “Need to set up your account?” – username (your email address) and password (you create) will be needed.
Registering For The Patient Portal
After your portal is validated, you will receive electronic forms that will need to be completed 72 hours before the set appointment. The patient forms are lengthy so please press save if you need to revisit and complete at a later time. These forms are important for the doctor/nurse to see your history and to better understand your needs.
Your patient portal will be your direct link to viewing your next appointment, sending a secure message to the doctor or nurse, REQUESTING REFILLS, etc.
