Fee Schedule
We are now contracted with Blue Cross Blue Shield, Cigna, United, and Aetna, so our current fee schedule does not apply to customers on Blue Cross Blue Shield, Cigna, United, or Aetna policies. For all others, at the time of service, we will provide you with a Superbill for you to submit to your insurance company. Our fees are based upon the complexity of your appointment and the time spent with your Provider. The fee schedule is as follows:

| Adult Paperwork (Guardianship, FMLA, Disability) | $ 25.00 |
| More Complex Letters | $ 40.00+ (Depending on Complexity) |
| Provider Legal Fees (chart review, in-court appearances, communication between provider and patient legal counsel) | $ 450.00/hour rounded to the nearest hour due at the time of service + Invoice cost from practice’s legal counsel |
We accept cash, MasterCard, Visa and Amex. Please bring your Insurance Card and Driver’s license (or picture ID) and prescription card with you. Your Insurance card will be used for the purpose of doing prior authorization for certain medications only and not for medical billing purposes.
